Is there any existing module: 125Khz RFID module - UART, I need this for its 150mm maximum distance. The other model on, Electronic brick - 125Khz RFID Card Reader, just 70mm maximum distance. Thank you.

The module work fine (or at least I guess). I have bought tags sold on the website and I have signal which change on the led pin (I have not yet try to develop and arduino application - it takes time to less time to check at hardware level).
Based on the information that the module is ok and that my HID tag works at 125KHz too, I do not understand why.

I have checked with an oscilloscope what it happens on the antenna when my HID RFID 125KHz tag is close of the reader and I can see nothing special.
I am sure that it is 125KHz tag since I have tested the signal used by readers for access control of my building with an oscilloscope (and the specification said that it is 125KHz).
Is there something that I can check on the module to be sure that it have not a problem?

Okay, please make sure that you are using RFID 125Khz card such as(http://www.seeedstudio.com/depot/rfid-tag-combo-125khz-5-pcs-p-700.html?cPath=144_153). If so, it doesn't work ,it is maybe has a broken. You could directly contact our QC( QC@seeedstudio.com).

Dear Deray,
When I put my HID RFID 125KHz tag, the output level on LED pin is not moving (and stay HIGH).
I really wonder if the module has not a problem.

Hello, you could directly measure the voltage of LED pin. If there is not RFID card , it will always output HIGH level, otherwise it output LOW level.
Nothing is said about how to choose output mode.
I have one and it seems I can get nothing on the TX PIN of the reader ... :-(
Hello, this module is default to set as RS232 output mode after manufacture , it can not change the output mode. Please check if you are using 125k RFID card , the TX port will reply the ID card when a 125k RFID card is put close to it.

Was something changed on this module recently? The last one I got is certainly different from the first (different layout of components, pins look the same). In particular, I am no longer getting +5V on pin 5 of P1.

Got mine the other day and played around with it. I'm getting consistent data at this point, but it isn't matching the serial numbers on the tags. I agree with the other reviews, the antenna seems very fragile. I'm wondering if you could build your own antenna and increase both the range and durability. Overall, this is a great RFID board for the price.
The "Crazy People" interrupt based code is probably the best place to start if you're going to use this with an Arduino.
